If resident of long-term care or assisted living facility, is the facility capable of managing the acute illness?

For patients in independent living, can they be cared for at home?

• Home care services (RN, PT, OT) for post-ED follow-up, monitoring at home and/or evaluation of home environment
• Primary care provider outreach for early follow-up
