Building on this momentum, The John A. Hartford Foundation recently announced $11.4 million in new grants to accelerate innovation and expand age-friendly practices across care settings.

Recent examples shared by the Emergency Nurses Association (ENA), demonstrate what it looks like to integrate the 4Ms into everyday workflows. These real-world examples show how teams are improving care for older adults in meaningful ways, with some familiar organizations and leaders helping drive this progress.
