# Treat & Release Toolkit

What's Inside?

Treat & Release is a tool designed to ensure patients who are sent to the emergency department from long-term care facilities (LTCs) or skilled nursing facilities (SNFs) can quickly receive the care they require in the emergency department, and, ideally be sent back to their living environment as quickly as possible. By implementing this tool in your ED with the network of SNFs or LTCs in your network, you can improve communication and provide better care for high-risk, complex older adults in your ED.

Provided here is the tool, suggestions for customizing the tool on a needs basis, a brief online training for both EDs and SNFs/LTCs, and a deck to present to key stakeholders to promote buy-in.
