Key Points

The measure provides financial incentive and support behind improving care for older adults.

The overall aim of the CMS Age-Friendly measure is to create a system that benefits patients, physicians and hospitals alike.

Domain 1: Eliciting Patient Healthcare Goals. Connecting with patients to find out their own goals of care and what matters most to them. It also provides an extra touch point for the system to check bigger picture care plans: code status, life sustaining preferences, surrogate decision makers, advanced directives.

Domain 2: Responsible Medication Management. Having a nuanced approach and deeper understanding of medications is essential for physicians when considering medication management.

Being attentive to deprescribing practices, polypharmacy, and factors impacting dosing in older adults.

Caution when prescribing medications that are potential triggers of delirious episodes.

Potential for development of speciality specific guides for commonly prescribed medications.

Concern that there may be unintended consequences for clinicians due to indiscriminate flagging tools (eg. the AGS Beers Criteria).

Domain 3: Frailty Screening and Intervention. Screening needs to be used in a meaningful way to create management plans that address patients’ needs. Rather than an additional burden, this is an opportunity to capitalise on pre-existing hospital resources and data collection methods.

Domain 4: Social Vulnerability. The ED can be used to screen and intervene for a broad range of social vulnerabilities. The earlier these are identified, the better, as care plans for a patient can be created accordingly. A MDT and Community specific support is key to this domain.

Domain 5: Age Friendly Care Leadership. Having a designated champion, committee or team is necessary to ensure the other 4 domains are being implemented. There should be initial and ongoing input from a variety of stakeholders, a clear reporting structure and chain of command, and hospitals need to make resources available to leadership so goals can be met.
